Discharge Planning and Community Transition involves coordinating a safe move from hospital or rehabilitation services into home or community care. Assessment considers ongoing treatment, functional ability, housing, equipment, rehabilitation and support needs, alongside risks associated with discharge. Independent expert witnesses can evaluate the adequacy and timing of discharge arrangements, communication between services, continuity of care and resulting harm or additional needs in Clinical Negligence and Personal Injury proceedings.
